The Health Information Management Department (Medical Records) is open to the public from 8:00 a.m. to noon and 1:00 p.m. to 4:00 p.m., Monday thru Friday for anyone wishing to obtain a copy of their medical records. All requests for records will require a 24 hour advance notice. You may call the HIM office at (575) 546-1315 in advance to have your records ready for pickup. Proper photo identification is required prior to release of records.

We strictly adhere to HIPAA rules and regulations for the protection of your privacy.

Advance Directive forms are available in the HIM Department. They do not have to be witnessed to be valid in the State of New Mexico. If you wish to have them notarized, there is a Notary Public available.
