The transition from hospital to home can take time. Mimbres Memorial Hospital’s Skilled Nursing Care Facility provides continued care for people going through this change. Here, our supportive nurses offer a variety of caring services and support that help people and their loved ones get to a place where they are ready and prepared for a successful return home.
Our mission is to provide comprehensive, appropriate post-hospital medical and rehabilitative care for patients who have experienced trauma or severe illness. Through carefully planned rehabilitation, patients can recover or increase their ability to perform activities of daily living. This may involve re-learning language or speech, regaining motor skills, coordination and memory or recovering strength and stamina through good nutrition and exercise. For some patients, rehabilitation may also include the adoption of compensatory skills that will allow them to function more independently on their own.
Our professional team works with the patient and his or her family on a treatment plan that will meet each person's unique needs. This is accomplished through a comprehensive patient assessment, establishing therapeutic goals, close monitoring of patient progress and the additional contribution of wound care and patient education on medications and disease processes. A registered clinical dietician will also evaluate and recommend appropriate nutrition.
